School assessment referral checklist.

Use this checklist to organize the referral question without diagnosing the student.

Define the question

  • What specifically needs clarification?
  • What decision will the assessment help inform?
  • Which settings or tasks show the concern?
  • What strengths should be understood alongside the difficulty?

Describe functional impact

  • learning and academic work
  • attention, organization and task completion
  • participation, communication and relationships
  • emotional or behavioural functioning
  • attendance, transitions or daily school routines

Summarize what has been tried

  • instructional changes
  • targeted intervention
  • accommodations or environmental supports
  • consultation with school or external professionals
  • the observed response and timeframe

Identify potentially relevant information

Share information only with appropriate consent and through an approved secure method.

  • progress information or work samples
  • school records requested by the clinician
  • previous assessment or intervention reports
  • specific teacher observations
  • questionnaires or consultation requested by BrainLab

Before sending the referral

  • Avoid stating that a student has a diagnosis unless it is already established and relevant.
  • Confirm consent and the appropriate contact route.
  • Do not email sensitive records until BrainLab provides secure instructions.
  • Note any time-sensitive external documentation requirement without assuming it can be met.
