Adult PHQ-9 depression questionnaire.

A nine-item measure of depressive symptoms over the past two weeks. It includes a question about thoughts of death or self-harm, is not monitored and cannot assess suicide risk or establish a diagnosis.

What this measure assesses

The PHQ-9 asks adults about nine depressive symptoms over the past two weeks and takes about three minutes.

Safety and privacy

Any response other than “Not at all” to Item 9 immediately displays 9-8-8 and emergency guidance. No response triggers a message, notification or record. Item 9 alone cannot classify suicide risk, and a zero response does not prove that risk is absent.

Intended for: Adults age 18 and older

Timeframe: Over the last two weeks, how often have you been bothered by the following problems?

Choose one response for each item. This questionnaire includes a question about thoughts of death or self-harm.

1. Little interest or pleasure in doing things
2. Feeling down, depressed, or hopeless
3. Trouble falling or staying asleep, or sleeping too much
4. Feeling tired or having little energy
5. Poor appetite or overeating
6. Feeling bad about yourself—or that you are a failure or have let yourself or your family down
7. Trouble concentrating on things, such as reading the newspaper or watching television
8. Moving or speaking so slowly that other people could have noticed—or the opposite, being so fidgety or restless that you have been moving around a lot more than usual
9. Thoughts that you would be better off dead or of hurting yourself in some way

Screening vs assessment

A questionnaire is not a psychological assessment.

Screening can support reflection, but it does not diagnose, produce a psychological assessment report or replace professional evaluation. You do not need to complete a screener before requesting an assessment.

Does the PHQ-9 diagnose depression?

No. It measures recent symptom frequency and requires professional context for diagnostic use.

Does Item 9 determine suicide risk?

No. A positive response is important and deserves support, but the item is not a comprehensive suicide-risk assessment.
